CMS Revises Medicare Provider Enrollment Forms
In July 2011, the Centers for Medicare and Medicaid Services (“CMS”) released new versions of the forms that providers, including health centers, are required to use for Medicare enrollment. Health centers must submit CMS Form 855A to enroll in Medicare as a federally qualified health center.
Among other changes, the new version of Form 855A increases the amount of information a health center must submit about its Board of Directors and managing employees. CMS revised the form as part of its continuing efforts to prevent individuals and entities that have been excluded from participation in federal health care programs from re-enrolling in the Medicare program.
